Yeah, just marker.. Most tattoo place won't tattoo you until your 16 even if you have a parent's consent.. It may be realy though

i'm sure it's marker cuz in a pic (don't remember wich one) she AND miley have a heart exactly in the same place... and miley was signing autographs and she had a pen... so i think she likes to paint hearts in her mom's hand lol xD
