I used Photoshop Elements 2.0, I'm not sure if this is translatable for other programmes,
because I don't know if there is the Colour Variations option

1. Click Auto Contrast (Alt+Shift+Ctrl+L) and sharpen

This will be our base

2. Create new layer, and fill with #02214F, set to exclusion

3. Duplicate base, drag to top, desaturate, and set to softlight

4. Flatten, go to Enhance> Adjust Colour> Colour Variations> Midtones, and decrease red to your liking

5. Duplicate base twice, set the first layer at screen, and the one on top at soflight

6. Flatten, create new layer, and fill with #02214F, set to soflight

7. Adjust Brightness/Contrast levels, increase contrast by 10

You might want to lower the opacity of the Screen layer in step 5 ;) Miley just looks a little bright :P Also you don't need to add contrast if it isn't necessary ;)

Remember to adjust it to your image.
